Getting Started on ELAPRASE

Step 1: Discuss OnePath support services with your patient and their family. We suggest passing along the OnePath patient brochure that explains how OnePath may offer assistance to patients about to start treatment with ELAPRASE.

Step 2: Fill out the Start Form completely, and make sure you sign it. In addition, please complete the Statement of Medical Necessity form. If your patient (or the patient’s guardian) is interested in receiving product support services, please ask the patient/guardian to sign the Start Form. Patients are not required to sign the Start Form to receive ELAPRASE therapy but must do so if they would like access to OnePath support services.

Step 3: Fax the signed Start Form, Statement of Medical Necessity form, and a copy of your patient's insurance card(s) to OnePath at 1-888-990-0008.

Step 4: If your patient (or their guardian) has signed the Start Form, a OnePath case manager will contact them.

Indication
ELAPRASE® (idursulfase) is indicated for patients with Hunter syndrome (Mucopolysaccharidosis II, MPS II). ELAPRASE has been shown to improve walking capacity in these patients.


Important Safety Considerations


Risk of anaphylactic reactions: Life-threatening, anaphylactic reactions have been observed in some patients during ELAPRASE infusions. Therefore, appropriate medical support should be readily available when ELAPRASE is administered. Biphasic anaphylactic reactions have also been observed after ELAPRASE administration and patients who have experienced anaphylactic reactions may require prolonged observation. Patients with compromised respiratory function or acute respiratory disease may be at risk of serious acute exacerbation of their respiratory compromise due to infusion reactions, and require additional monitoring.
